By signing below I, on behalf of myself and my child, agree to defend, hold harmless, indemnify, waive and release The Jewish Center, as well as its directors, officers, administrators, board, employees, and other agents from any and all claims, costs, demands, suits, liability and damages, including without limitation those based in negligence and all consequential damages, for any and all injuries or harm arising directly or indirectly from any synagogue sponsored trip or activity in which my child participates. I acknowledge that participating in trips and activities may present risks to my child and her or his property that are different from and greater than those associated with attending synagogue or youth groups and events, and that The Jewish Center’s (“The JC”) insurance does not cover damages arising from, or related to, trips or activities that take place outside The JC building, the operation of any public or private vehicle, or any personal negligence related thereto.
In addition, I represent that I have spoken to my child about the standards of appropriate behavior and conduct expected of him or her while participating in synagogue sponsored trips and activities. My child shall abide by all rules and instructions of faculty or other adult supervisors and shall not engage in inappropriate conduct, including the use of physical or verbal violence, consumption of illicit substances, or open abuse of the customs or mores of the community. The JC or its representative may, in its/his/her sole discretion, terminate my child’s participation on any trip or activity at any time and for any reason, including not limited to: inappropriate conduct or other behavior deemed detrimental to the best interests of The JC; failure to follow the instructions of faculty or adult supervisors; emergencies; or health or safety considerations. Such termination shall not diminish or otherwise alter my obligation to make any payment(s) required, nor shall The JC be required to make any refund.
